    Default GA-Z77M-D3H and PCIe 16

I'm leaning on this being a software conflict, but could have been mobo related.

    Win7 Pro/64bit....8GB DDR3....EVGA GT-520/1GB video card (PCIe)

I got a black screen, then a Windows recovery of the video driver. Upon restart the system would no longer see the video card in Device Manager. EVGA sent another card, although after 48 hours when it did it again, I was now fairly certain it's not strictly the card.

I could boot with internal graphics fine, the system did not see the card. Then I put the card in the PCIe 4 slot, and it was recognized. Upon EVGA's thoughts as well, I then did a CMOS reset and went ahead and flashed with the F14d beta BIOS from the Gigabyte page. Card is again being seen in the PCIe 16 slot, and all appears working normally.

A BIOS issue? A Simple need for a CMOS reset? Power supply tests OK (I used a small proprietary power supply tester, not sure of their reliability). Power Supply is a good Corsair 600w model.

What say you? History of a video driver stopping, and card vanishing from the Device manager?

So far, yes, it is working. While normally not a fan of Beta BIOS, because of the issue I flashed with F14d. When the final release of new BIOS is released, I'll flash with that, and hopefully close the book.
