Need help with weird issue.

Hi guys! For the past few months, I've been experiencing some very noticeable texture pop in and shadow flickering in game. Every time I observe a shadow, it'll blink rapidly and whenever I run towards objects, they'll appear out of nowhere. This happens in nearly every game I've played, Assassin's Creed Brotherhood, Team Fortress 2, Bad Company 2, and Crysis 2 all had noticeable levels of texture/object pop in. As well as jagged shadows even with AA on. I have a 5850, 11.8 drivers, and 32 bit Vista. How do I fix this? Thank you.
